2022 Employee Recognition Nomination Form

Each year at our annual banquet, we recognize employees who exemplify our corporate values. Please use this form to nominate those in our organization who you feel have gone above and beyond in demonstrating our values: Taking Ownership, Promoting Teamwork, Driving Improvement, Respecting and Appreciating Others, and Living the Mission. Nominees can be individuals, project teams, departments, or locations. If you have questions regarding eligibility, please contact Janet Lowrey at email.

Goodwill’s 2022 Recognition Banquet will be held on Thursday, November 17 at Benton Convention Center in downtown Winston-Salem. A social hour begins at 6 p.m. and the program/dinner begins at 7 p.m. In addition to employee recognition, Goodwill will honor board members, volunteers, agency partners, and program participants. All Goodwill Northwest NC employees will receive an invitation and additional details in September.

Tips for submitting your nomination:

  1. Refer to Goodwill’s Values and Behaviors as you consider your nominee.
  2. Explain how your nominee demonstrates the value on a regular basis (not just a one-time situation) and how they go above and beyond in modeling that value in their daily work and interactions with others.
  3. Include examples of how the nominee demonstrated the value.
  4. Incomplete submissions will not be considered.

Deadline for submitting nominations is Friday, July 8, 2022.
